All students participating in band - including returning band students - need to have a completed health form for the 2009-2010 school year. This form can be found on the band’s website via this link 2009-2010 Health Form (opens PDF file in a new window) and must be returned PRIOR TO THE FIRST DAY OF BAND CAMP. The health form should include a copy of your health insurance card and a copy of the student's immunization record (do not let the copy of the immunization record prohibit you from turning in the form).

Please be sure to list all medical information including ANY allergies or other health issues/concerns that our chaperones and band directors need to be aware of.

Health Forms can be mailed to the Fort Mill District Band Booster Club, P.O. Box 611, Fort Mill, SC 29716 or dropped off in the mailbox just inside the band director's office.
