Please remember that all students need to have a completed health form (can be found on the band’s website via this link 2008-2009 Health Form) PRIOR TO THE FIRST DAY OF BAND CAMP. The health form should include a copy of your health insurance card and a copy of the student's immunization record (do not let the copy of the immunization record prohibit you turning in form).

Please be sure to list all medical information including ANY allergies or other health issues/concerns that our chaperones and band directors need to be aware of.
